School Based Mental Health Practitioner- $2000 sign on bonus

Fernbrook Family Center's School-based CTSS program is located within Southern Minnesota Education Consortium which provides mental health services within the school setting. These services are provided within a federal Level 2/3 setting determined by student’s Individualized Educational Plan (IEP). Students within this setting spend part of their day within a Learning Center Resource classroom and transitioning to mainstream classes throughout the school.

School-based CTSS programs serve students whose mental health concerns present a barrier to special education services. Fernbrook’s role is to help maintain and regulate student behaviors. Staff will provide individual and group skills focusing on mental health and mood regulation strategies. Staff follow the school’s schedule and will work collaboratively and effectively with school personnel.

Fernbrook is seeking to add a full-time School Based Mental Health Practitioner to work within Fillmore, Mower and Freeborn counties. The services are time-limited interventions that are delivered using various treatment modalities and combinations of services designed to reach treatment outcomes identified in the individual treatment plan.

Requirements:

  • Must have either one of the following to qualify:
  • Master's degree in a behavioral service/related field OR
  • Bachelor's degree in a behavioral service field and 2000 hours working with children under 21 years of age with a mental health diagnosis OR
  • 6,000 supervised hours working with children under 21 years of age with a mental health diagnosis * Must be available to work some evenings per week
  • Excellent communication, advocacy and collaboration skills preferred
  • Must have reliable transportation, a valid driver's license, valid automobile insurance, and acceptable driving record
  • Must pass criminal background check based on agency and Department of Human Services requirements

Responsibilities:

  • Teach and provide school and in-home and community based skills training
  • Enable clients to improve basic functioning of daily and community living activities
  • Manage own work schedule with support of supervisors; schedule sessions with client caseload
  • Create & complete individual treatment plans for children & adults in timely manner
  • Create progress notes 24 hours after client appointments
  • Attend & actively engage in individual supervision and all staff meetings
  • Create effective working relationships with Fernbrook staff, school staff, care givers, and community partners
  • Other duties as assigned
