Responsibilities:

·          Responsibilities include utilization review, medical social work and discharge planning functions.

·          Evaluates patient needs and assists with developing a plan for post-hospital care.

Requirements:

·         Excellent communication skills needed.

·        Includes On-Call and Weekend hours. 

·         Must be able to work independently and collaboratively with other members of the healthcare team.

·        Bachelor's degree in social work, psychology, human services or nursing required.

·       Current VT RN license required.
